Please consider the Pastor’s work during the evaluation period. Rate only what you can reasonably assess; choose “N/A — Not observed” when you do not have enough information.
Your access code opens the questionnaire prepared for your church.
Your feedback is stored confidentially with encrypted transmission and storage. Reports are restricted to the authorized evaluation committee. Authorized service administrators maintain the system and may access stored data when needed. Your code can be submitted once. Code-use records are kept separately from your answers. Comments may identify you, so please avoid names and identifying details.
